Benefits of Our Comprehensive Management Care Program

• Clients are assigned to an RN and/or Service Coordinator as their Alpha Care Hawaii Corp. manager who is responsible for overseeing all care needs.
• An evaluation which includes physical, mental, medical, social, environmental and financial status and development of a client’s dedicated care plan, in conjunction with client and family.
• As experts in the health care field, our nurses or caregiver will act as patient advocates who will assist and provide a voice in matters of health care and coordinate needs with an Alpha Care Hawaii Corp.
• Help with understanding and utilizing available resources in the community.
• Ensuring that appropriate professionals are meeting the required health needs of the client.
• Observation of the client, and reporting status updates to family members and Alpha Care Hawaii Corp.
